This builder sets the parties and the certification for the contractor to subcontractor relationship, in the style of the G702S subcontractor version. You show the contract sum, the work completed and stored to date, the retainage the GC holds, previous payments, and the current amount due.
The continuation sheet breaks it down by your schedule of values. If your contract uses a different retainage rate per line, you can set that too.

Does it handle retainage the GC holds?
Yes. Retainage on completed work and stored materials, total earned less retainage, previous payments, and current payment due, with per-line rates if your contract varies them.
Is this an official AIA form?
No. It follows the same math and structure contractors know from AIA-style billing, but it is our own format. If your contract requires official AIA documents, purchase them from AIA.
